Important Information

Prescription Medications
Prescription requests will only be filled during routine office hours.

We suggest that you call your pharmacy for prescription renewals at least three days before your current supply runs out. Our Physician will review your history and return the faxed request to your pharmacy with the appropriate authorization.

We also suggest that you call ahead to confirm that your prescription is ready before you go to your pharmacy. If you use a mail-order prescription service, please ask them for your refills at least two weeks in advance. This will usually avoid any lapse in your medication.

Referrals to Specialists
Our Physicians will coordinate any referral you may need with a medical specialist. We will strive to work within the scope of your insurance plan and refer you to a specialist who also participates in that plan. Please do not make an appointment with a specialist until you have received a referral form, it cannot be generated after your appointment with the specialist.

Test Results
All lab/test results are reviewed by our doctors. Our office will then contact you two to five days with the results and indicate any further course of action. If you have not been contacted about your test results within five business days, please contact our office.

Medical Insurance
Please familiarize yourself with your insurance requirements and coverage allowances, and bring your insurance card with you to each appointment. We participate and accept assignment of benefits in a variety of insurance plans.  Any deductibles or non-covered charges may be paid by cash or credit card at the time of your visit.

If you have any questions regarding our services or fees, you are welcome to discuss them with us. Should unusual circumstances require special arrangements for payment of your bill, please bring this to our attention; our billing office number is (301) 830-3940.

Medical Records and Forms
We do not release copies of your medical records to anyone without your written consent. If you require a copy of your own medical records, we will need your request in writing.
